Patient Statements

We’ve partnered with Avadyne Health, a trusted Iowa-based leader in hospital receivables management, to handle statements for all our services.

Here’s how it works: After your insurance pays its part, your account details are sent to Avadyne. They will then send you a statement for any remaining balance you owe. This new approach ensures that your statements are more user-friendly and easier to understand. Additionally, an Avadyne representative will reach out to you to clarify your bill, answer any questions, and help streamline payment plans for your convenience.

Financial Assistance

At LCHC, we recognize that managing medical expenses can be challenging. That’s why we provide financial assistance to eligible individuals. To apply for financial assistance, please follow these steps:

  1. Download and complete our Financial Assistance Application.
  2. After completing the necessary forms, please return the application along with the required documents, such as your most recent filed tax returns and income documentation for the past three months to the following address:
