LCHC Announces Changes to Sports Physicals Procedures

Lucas County Health Center is committed to ensuring the health and safety of our community’s student athletes. In alignment with recent updates from the Iowa High School Athletic Association, we are updating our sports physical procedures to better identify health concerns and ensure safe participation in athletic activities.

Starting immediately, all sports physicals for student athletes must be scheduled with a Primary Care Provider (PCP). This change is designed to provide a more thorough and personalized health evaluation, ensuring that each student is fit for participation.

To facilitate this process, the Iowa Athletic Pre-Participation Physical Exam Form must be completed prior to the appointment. You can obtain the form on LCHC’s website or at Chariton High School during their summer hours. Please ensure that pages 1-2, including signatures from the parent/legal guardian and the student, are completed prior to the exam.

Parents should also plan to accompany their student athlete to the appointment. Parent/guardian consent is necessary to initiate the exam and an adult presence ensures that all parties are informed and involved in the process.

The cost of the sports physical will be billed to your insurance, and office visit co-pays will apply. It is important to check your coverage details.
