Lucas County Health Center is pleased to announce the availability of Advance Directives Packets, now available for pick-up at the hospital front desk and the clinic front desk. We invite all community members to take advantage of this valuable resource to ensure their medical preferences are known and respected in times of emergency.
The Advance Directives Packet includes:
- Advance Directives and Living Will: Complete the “Gift of Peace of Mind” packet and deliver a copy to your primary care provider or hospital. These documents will be added to your medical record. Additionally, complete the IPOST form, take it to your next primary care appointment for your provider’s signature, and request it be added to your medical record. Use the large red folder to store the originals in a safe, easily accessible place for your loved ones.
- Fridge Magnet: Fill out the “Medic Take Note” form and secure it to your refrigerator door. We recommend adding copies of insurance cards or other essential medical documents to this pocket folder for easy access by emergency medical personnel.
- Vehicle Info: Complete the “My Medical Info” form and place it in the smaller red “Medical Information” folder. Store this folder in your vehicle’s glovebox, center console, or another secure location.
- Medical ID Wallet Pal: Fill out the “Medical ID” wallet pal and insert it into your wallet or purse. Ideally, place it near your license or government ID. In case of emergency, medical personnel will look for this first.
- Stickers: Two “My Medical Information” stickers are included to help emergency medical personnel or loved ones find important information. These stickers can be placed at an entry point of your home, in your vehicle, on a filing cabinet, or other useful places. When filled out, they should read, for example, “My medical information is in the filing cabinet in the office.”
