In alignment with recent updates from the Iowa High School Athletic Association, we are updating our sports physical procedures to better identify health concerns and ensure safe participation in athletic activities.
All sports physicals for student athletes must be scheduled with a Primary Care Provider (PCP). This change is designed to provide a more thorough and personalized health evaluation, ensuring that each student is fit for participation.
Sports Physicals Step-By-Step Guide
- Download the Pre-Participation Physical Examination Forms and complete pages 1 and 2, including signatures from the student and parent/guardian. This must be done prior to the appointment. (Forms can also be picked up at Lucas County Health Center.)
- Check insurance coverage. (See information below.)
- Schedule an appointment with a Primary Care Provider. A parent/guardian should plan to attend this appointment to ensure all parties are informed and involved in the process.
Insurance Information
The cost of the sports physical will be billed to your insurance, and office visit co-pays will apply. It is important to check your coverage details. If you have questions about insurance coverage, contact LCHC’s Business Office Manager at (641) 774-3000. We are committed to working with families and students who may need assistance to cover the cost of the physical.
In the future, families should consider coordinating insurance-covered yearly physicals with their annual sports physical.
