Days and Shifts:
- Full-time position
- Mainly Monday – Friday from 8 AM – 5 PM
Description:
- Assist the Clinic Team (providers, nurses, support staff) by coordinating care to patients on the providers’ daily schedules and by proactively managing and coordinating care for patients not on the schedule, so as to offer quality healthcare to all clinic patients
- Will act as a primary conduit for the transmission of information between providers, patients, and medical record
- Will coordinate services for patients, including preventative care, health education, and advocacy
- Focuses on promotion of health, prevention of illness and injury, and protection from work-related and environmental hazards
- Delivers health and safety programs and services to workers, worker populations, and community groups
Requirements
Educational/Experience:
- Current Iowa Licensed Registered Nurse, LPN or CMA
- Previous experience in a medical office
- Healthcare Provider BLS certification
- ACLS certification
- Mandatory Abuse Reporter certification required within 6 months of employment
- Ability to read, write, speak, and understand English well
- Computer literate – Microsoft Office and able to learn additional software programs
PREFERRED:
- Achieve and maintain certification in Integrated Care Management within 6 months of hire
- Previous process improvement experience
- Previous experience in Care Coordination
- Certification in Occupational Health Nursing (COHN or COHN-s)
- Certification in Hearing conservation (CAOHC) required within 6 months of hire
- Minimum of three (3) years nursing experience, particularly in community health, ambulatory, critical, or emergency areas
- Experience in occupational health nursing and follow-up care preferred
