The role of the Care Coordinator is to assist the healthcare team including medical providers, patients, and families by coordinating and facilitating patient care leading to the best outcome while providing regular and consistent follow-up during transitions of care with patient, coordinators and other healthcare professionals in other care settings.

Responsible for providing direct care to those in need.  Perform nursing assessments and assist physician with employment and surveillance examinations.  Educate individuals on health/wellness/safety related topics through individual discussions or group training.  Communicate effectively with other peers for continuity of care; patients for injury care instruction; and others in personnel and safety with a need to know.  Maintain satisfactory relations with other departments and units.  Maintain confidential electronic medical records.  Maintain satisfactory relations in accordance with mission, vision and core values of Lucas County Health Center.

  • Under limited supervision, plans, directs, and evaluates total nursing care and functions in one unit on a particular shift, including functioning in Charge Nurse role. Responsible for orientation, training, and management of personnel.  Relates effectively with other shifts for continuity of care; maintains satisfactory relations with other departments; participates in all phases of education, maintenance of records, and upgrading of policies, procedures, and skills.
  • Administers medications, injections, intravenous solutions, chemotherapy agents skillfully and correctly.
  • Assists in outpatient clinics, specialty clinic, infusion clinic/diabetes education, cardiac/pulmonary rehab, occupational health, and wound healing clinic.
  • Demonstrates competencies in diabetes self management education including but not limited to Diabetes Basic Overview, medication/glucometer testing, diet management, and diabetes wellness.

Days and Shifts:
Mainly Monday – Friday 8:00am-5:00pm

LCHC Medical Clinic

Current Iowa Licensed Registered Nurse
Previous experience in a medical office
Healthcare Provider BLS certification
ACLS certification
Mandatory Abuse Reporter certification required within 6 months of employment
Ability to read, write, speak, and understand English well
Computer literate – Microsoft Office and able to learn additional software programs

Achieve and maintain certification in Integrated Care Management within 6 months of hire.
Previous process improvement experience.
Previous experience in Care Coordination.
Certification in Occupational Health Nursing (COHN or COHN-s)
Certification in Hearing conservation (CAOHC) required within 6 months of hire
Minimum of three (3) years nursing experience, particularly in community health, ambulatory, critical, or emergency areas
Experience in occupational health nursing and follow-up care preferred
