Cardiac Rehabilitation
LCHC Cardiac Rehabilitation multidisciplinary team is specially trained to treat people suffering from cardiac disease or those who are recuperating from surgery.
Patients who believe they are in need of services offered by LCHC Cardiac Rehabilitation are encouraged to consult with their physician. Family doctors and specialists will refer patients to LCHC Cardiac Rehabilitation for treatment.
LCHC Cardiac Rehabilitation provides an individualized plan of care specific to each individual’s needs including:
- Medical evaluation
- Monitored exercise
- Home exercise program development
- Lifestyle/risk factor assessment and modification options
- Medication teaching
- Diet instruction
- Client and family educational counseling and behavioral interventions
