Cafeteria Plan - Flexible Benefits
Full-time employees may participate in the flexible benefits plan. The plan includes pre-tax payroll deductions for health and dental insurance, as well as day care and health care flexible spending accounts.
Health and Dental Insurance
Health and dental plans may be purchased by full-time employees with premiums paid through pre-tax payroll deduction. If eligible, this coverage is effective the first day of the month following 60 days after your start date.

PTO Accrual
PTO accrual is based on the total number of hours paid up to 80 hours per pay period. Employees shall earn PTO in accordance with the following schedule and may accumulate paid time off up to a maximum limit of 320 hours.

Shift Differential
PM Shift Differential:
From 6:00 p.m. to 12:00 midnight 10% above the hourly rate
Night Shift Differential:
From 12:00 midnight to 6:00 a.m. 20% above the hourly rate
IPERS
Iowa Public Employees Retirement System -- Each employee contributes 3.7% of their gross earnings and the hospital contributes 5.75%. This benefit does begin with your date of hire.
Life Insurance with Accidental Death & Dismemberment Coverage
Life insurance for full-time employees is valued at $15,000, $2,000 for a spouse, and $2,000 for each eligible dependent at no cost. Additional life insurance coverage is available at the employee's expense. If eligible, this coverage is effective the first day of the month following 60 days after your start date. Full-time employees are covered by Accidental Death & Dismemberment Insurance, at no cost, according to the policy carried by the health center. If eligible, this coverage is effective the first day of the month following 60 days after your start date.
Additional Guarantee Issue Group term life insurance with accidental death and dismemberment provisions may be purchased by full-time employees with premiums paid through payroll deduction. Employees may apply for any amount of coverage up to $70,000 and any amount of coverage up to $25,000 on spouse as well as coverage up to $10,000 on dependent children. Disability
Short- and long-term disability is provided for full-time employees. This coverage is effective the first day of the month following 90 days of employment. This coverage is offered at no cost to the employee with full premiums paid by LCHC. The disability program pays 50% of your wages (up to a maximum benefit of $1,500 per week) beginning the first day of absence due to injury or on the seventh day of absence due to illness.
Compassionate Leave
Full-time employees may receive three 8-hour days of compassionate leave in the event of death in their immediate family. Part-time employees may receive one 8-hour day of compassionate leave in the event of death in their immediate family. This benefit does begin with your date of hire.
Tax Sheltered Annuity
The health center administers a program of payroll deductions and investments on behalf of participating employees. You are eligible for this benefit the first day of the month following 60 days after your start date.
Financial Assistance for Education
Loans up to $500
Upon completion of 12 months of employment, regular full-time employees are eligible to participate in the tuition reimbursement program. Repayment is made in bi-weekly installments at a minimum of $20 per pay period through payroll deduction.
Educational Expense Reimbursement
Financial assistance up to $1,000 may be applied for by regularly scheduled full-time employees who have been actively employed for six consecutive months. Assistance up to $500 may be applied for by part-time employees working an average of 20 hours or more per week who have been actively employed for 12 consecutive months. Applications are subject to approval and available funding. Documentation of the completed course and paid receipt is required in order for the employee to receive this reimbursement. Applicants to whom assistance is granted will complete a term of not less than 12 months from the date of the expense reimbursement or refund LCHC the full amount of financial assistance through this program.
Forgivable Loans
This program offers financial assistance to individuals (employees or non-employees) who desire to pursue education in a profession requiring licensure/certification deemed “hard-to-fill”. Examples may include nursing, laboratory, radiology, physical therapy, pharmacy, etc., and will be identified and reviewed for their need on a case-by-case basis. A maximum of $7,000 may be awarded annually for the program. Two or more awards may be given annually. The maximum forgivable loan amount for any individual is $7,000. For every $3,500 recipients will be required to work full time at LCHC in the profession for which they complete their education for 18 months to meet the requirements of loan forgiveness. Amounts awarded for less than $3,500 will be pro-rated according to the amount of the award. Applications are subject to approval and available funding.
Child Care Center
LCHC operates a child care center for children from newborn to 12 years of age between the hours of 5:30 a.m. to 6:30 p.m. Monday through Friday. All LCHC employees receive a 10% discount per child. This cannot be used in combination with the multiple child discount.
